Changing the Hospital Type during Registration
The hospital type may only be changed during the normal registration period.
If the entity type is changed for an active hospital, the system will process the change as a new registration. Once approved by OPA, the system will terminate the existing registration on the first day of the next quarter and activate the new registration with the changed hospital type. Any existing child sites and contract pharmacy arrangements will be terminated when the old parent registration is terminated and new registrations and contract pharmacy arrangements must be created for the child sites.

If you select the Register as a different entity type option, the system will refresh the page to show the existing entity type, the New Entity Type drop-down list, and previously terminated hospital(s) with different entity type if available.

If you select the new entity type and click the Continue button then the system will display the Covered Entity Details page, allowing you to update the Employee Identification Number (EIN) field, if necessary, and to add billing and shipping addresses. From this point, proceed as usual for a new hospital registration.

Select the available previously terminated hospital type if you want to reinstate that specific entity and click the Continue button.
The system will display the Covered Entity Details page with the terminated entity's information allowing you to update the necessary information.
From this point, proceed as usual for a new hospital registration.
After submitting this submission to change the entity type to a previously terminated entity with a different entity type, you can submit the reinstatement of previously terminated outpatient entities under that entity or registration for new outpatient facilities.
